She didn't do so "immediately"? What did she do later? Was she trained to delay? Too bad she didn't just say she froze. Of course, this is a "reporter" giving the information. Who knows what she really said.

Who was responsible for training her to deal with multiple warnings? It's one thing if she panicked and froze, but it's quite another if company managers put her there knowing she wasn't adequately trained for the job.
Every light on my board goes red at once, alarms seem like a self-evident idea.
